The Learning Review: Adding to the accident investigation toolbox

Here’s another interesting read on the theme of learning, language and barriers from Crista Vesel, MSc and Ivan Pupulidy, PhD, discussing the background of the Learning Review.

The Learning Review first came about after adoption by the U.S. Forest Service, being “designed to facilitate the understanding of the factors and conditions that influence human actions and decisions by encouraging individual and group sensemaking at all levels of the organization”.

Not a summary – you can read this paper via the link; but some points from the paper:

·        The Forest Service’s previous SAIG manual displayed “straightforward cause-and-effect relationships, such as those developed in machines”

·        While these logics may be suitable for “strict mechanical problems”, it may not be “useful in human-centered work environments”; e.g. “People do not handle problems in a linear fashion—in fact, their solutions are often the antithesis of linear”

·        The SAIG also instructed for timeline-centric narratives, and not necessarily crafting an awareness of “complex human interactions”, which includes “balancing conflicting goals, messages, rules, regulations, direction, and even laws in their everyday encounters with complex work environments”

·        They then discuss the Learning Review approach – which I’m not covering here since you can fire up the paper yourself

It’s a good little read.
