One example revealed a major workaround that staff used every day because of poorly laid out equipment. The workaround worked, the job was successful, but the debrief exposed a latent system condition.

So in all: debrief the jobs that go well. Ask how they go well, what adaptations are needed, and how those conditions can be reproduced.
