Identification of management traits related to human factors in new views of safety approaches

This paper from Leônidas Brasileiro and colleagues evaluated common attributes across approaches grouped under ‘New View’.

i.e. HRO, S-II, SD, RE & HOP – they used both literature and Delphi method.

Not a summary since you can read the full paper. It’s an accessible read if you’re keen to quickly understand some key differences and similarities between these approaches.

Image 1 covers the core attributes across all included NV approaches.

They also highlight (images 2 & 3) how the different attributes overlap between the NV approaches [** Noting that RE would likely stream across the entire table if the whole body of literature was leveraged]

The most common attributes across all approaches were (accounting for 80% of total):

·        Anticipating issues in the working system

·        Flexibility in the system and towards risk

·        Accepting error as normal

·        Workers are influenced by leaders and peers (context matters)

·        Minimising blame

The remaining 20% was divided between Awareness of complex systems and control mechanisms, learning from failure, performance adjustment and leadership commitment.

Not surprisingly, “the research shows that the most represented NV approach is RE with 30% of hits, followed closely by SII with 27% of hits”.

That is, the vast majority of research from the broad NV guises sits under RE.

Based on the Delphi results, people saw the most important characteristics for safe systems as related to technical and organisational aspects. The “technical part is represented by the ability to anticipate risks in a system that must be flexible to absorb disturbances and recover fast”.

Anticipating risks are “built  into  a  system  by  providing  it  with  strong  defenses” , usually using a layered approach.

Flexibility is provided by “designing enough resources so as it has some built-in slack”.
